Nomination Form
The Canada-Poland Chamber of Commerce accepts self-nominations and nominations by members or non-members. Those nominated for an award must either be a CPCC member or of Polish descent to receive an award. We have five key award categories and suggested individual awards. Custom categories can be submitted to our Gala Committee for approval. Profiles of two or more nominees for one category will be assessed and chosen by the Gala Committee.
